Camp Forms for Registered Campers

Please note: The Parental Consent and Medical Forms are required for all campers. The Medicine Authorization Form is required ONLY if your camper will be taking medications during camp.

• Medical Form - This form must be completed by the child's physician and mailed or faxed into the address below by June 1, 2008. Any physical taken within two years of the start of camp is valid for our purposes and may be submitted in place of the attached medical form, as long as the information is consistent with what our form requires. If you have attended the Jim Calhoun Camp in the past we do not keep the medical forms, you will need to provide us with a new one. We cannot allow any youngster to participate until we have a copy of his physical. The fax # for the camp office is: 860-674-1704.
  
  
• Parental Consent Form [PDF] - This form must be completed and signed by the camper's parent/guardian and mailed to the camp office with your $150 deposit within two weeks of registering online.

• Medical Authorization Form - PLEASE READ THIS!
  This a STATE OF CONNECTICUT “Authorization for the Administration of Medications” form. One medication per form; EVERY medication (Over-the-counter (i.e. Advil) or Prescription (i.e. Albuterol)) must have its own form with the medication. This form must be signed by both your child's physician and child’s parent in order for the camp to administer medication to your child. PLEASE BE AWARE THAT WE CANNOT ADMINISTER MEDICINE - EVEN OVER-THE-COUNTER DRUGS WITHOUT THIS FORM BEING SIGNED BY A DOCTOR. Please do not mail this form to us. Please bring it with you on the first day of camp, signed, with any medications your child may require (please label all medications accordingly). Finally, SELF ADMINISTRATION AUTHORIZATION applies to Asthma and Epi-Pen medications only.
